About your information.

These questionnaires help Claim the Future Academy understand your interests and prepare to respond to your inquiry.

What is collected

Your name, email address and organization are required. Role and phone number are optional. CTFA saves your contact details when you begin, and saves answers as you select “Save & continue.” This allows staff to review inquiries and partial questionnaires as well as completed responses.

How it is used

CTFA uses these details to review your needs, manage follow-up and contact you about your inquiry. Providing a phone number allows a call about this inquiry; it does not subscribe you to marketing texts. This questionnaire does not subscribe you to a newsletter.

Access and storage

Responses are stored in the hosted survey application. The response dashboard is restricted to authorized CTFA access. Technical hosting providers process the information needed to operate the application. Browser cookies maintain your survey session for up to seven days on this device. They are not advertising cookies.

Your choices

Phone number is optional. You can ask CTFA to correct or delete your information by emailing info@claimthefutureacademy.org. Contact details may need to be confirmed before a request is fulfilled.

What to leave out

Please do not include employee names, claim numbers, medical records, Social Security numbers or confidential claim details. General descriptions are sufficient.
